VBS 2023

Consent/Release:
I give permission for my child to participate in the Spark Studios VBS program at Pleasant Hill Baptist Church of Gainesville, Ga. I give permission for my child to participate in the Twists & Turns VBS Games and Activities under the supervision of the VBS Leaders and Game Directors. I understand that my child is expected to follow the Code of Conduct of Pleasant Hill Baptist Church of Gainesville, Ga. and to behave appropriately. I release and wave, and further agree to indemnify, hold harmless or reimburse Pleasant Hill Baptist Church of Gainesville, Ga., the individual members, drivers, employees, and representatives thereof, as well as trip supervisors, from and against any claim which I and other parent or guardian, any sibling, the student unknown, directly or indirectly for any losses, damages, or injuries arising out of, or in connection with the student’s participation in any church VBS activity, or trip, or the rendering of emergency medical procedures or treatment, if any. I give Pleasant Hill Baptist Church the right and permission to use my child’s photograph in its promotional materials and publicity efforts. I understand that the photographs may be used in a publication, print ads, electronic media or other form of promotion. I release Pleasant Hill Baptist Church from any liability for any violation of any personal right I may have in connection with such use. By signing this consent, I state that I have read, understand and agree to the above consent/release.
